Weight Loss 3D™ Program — Effective Date: April 7, 2026

1. Agreement to Terms

By enrolling in the Weight Loss 3D™ program ("Program"), accessing our website (weightloss3d.com), or using any Weight Loss 3D™ services ("Services"), you agree to be legally bound by these Terms and Conditions. If you do not agree to these terms, do not enroll or use our Services.

Weight Loss 3D™ is owned and operated by M. Todd Wilson ("Coach"). Weight Loss 3D™ is a 100% virtual program. Correspondence address: 6430 East Main Street, Suite 101, Reynoldsburg, Ohio 43068.

2. Description of Services

Weight Loss 3D™ provides:

• Virtual wellness coaching focused on metabolic health and sustainable weight management
• Educational resources, meal planning guidance, and lifestyle strategies
• Supplement products (including Metabolic Reset Formula) available for purchase
• Access to our platform for tracking and communication

All Services are delivered online. There is no physical office location.

3. Scope of Services — Educational Coaching Only

Important: Weight Loss 3D™ provides educational wellness coaching and lifestyle guidance only. M. Todd Wilson is not licensed to practice medicine or provide medical treatment. This Program is NOT medical practice, does NOT diagnose or treat medical conditions, and does NOT replace medical care.

Before enrolling, you must consult with your doctor, especially if you have any medical condition, take medications, are pregnant, nursing, or have a history of eating disorders. Coaching includes general wellness strategies, education about metabolic health, meal planning guidance, and lifestyle recommendations. It does not include medical diagnosis, prescription of treatment, or management of medical disease.

You take full responsibility for any health outcome from following this program. Weight Loss 3D™ assumes no liability for health results or medical complications. Do not follow this program if your doctor has not cleared you. Stop immediately and seek medical care if you experience adverse symptoms.

4. Enrollment and Payment

• Final Enrollment: All enrollments are final. Once you complete enrollment and payment, your decision is binding.
• No Refunds: By enrolling in Weight Loss 3D™, you are entering into a wellness coaching relationship with M. Todd Wilson and the Weight Loss 3D™ team. This is a non-clinical, educational, and accountability-based program. M. Todd Wilson is acting as a health coach, not as a licensed medical provider. Due to the immediate allocation of coaching resources, program access, and product fulfillment, all enrollments are final with a $0 refund policy. For Affirm financing: Affirm's terms apply to financing disputes. Contact Affirm directly for financing questions. Weight Loss 3D™ refund policy applies to coaching and educational services only.
• Payment Methods: We accept payment via Stripe, Affirm financing, and other payment processors displayed at checkout.
• Financing: Affirm financing options may be available. Affirm is a third-party service; financing terms and fees are set by Affirm, not Weight Loss 3D™.

4A. Communications & SMS Consent

By enrolling, you consent to receive SMS text messages from Weight Loss 3D™ at the phone number you provide. Standard text message rates may apply. You can opt out of SMS marketing anytime by replying STOP to any message. We will honor STOP requests within 24 hours. You may still receive transactional SMS related to your appointment or program access.

5. Costs and Fees

Coaching packages and supplement prices are displayed on our website and during checkout. Prices may change. You will see the final cost before confirming payment.

6. Program Access and Platform Use

• Account Responsibility: You are responsible for maintaining the confidentiality of your login credentials. You agree not to share your account with others.
• Appropriate Use: You agree to use the Program and our platforms (website, app, GoHighLevel, and others) only for lawful purposes and in accordance with these terms.
• Prohibited Conduct: You may not:
  • Attempt to access unauthorized portions of our systems
  • Interfere with or disrupt our Services
  • Upload viruses, malware, or harmful code
  • Harass, abuse, or threaten other users or Coach
  • Attempt to circumvent security measures
  • Collect or use personal information of others without permission

7. Intellectual Property

All content on the Weight Loss 3D™ website and platforms (including text, graphics, coaching materials, meal plans, and proprietary systems) is owned by Weight Loss 3D™ or licensed to us. You may not reproduce, distribute, or use this content without permission.

Limited personal use of your own coaching materials for your own wellness is permitted. Sharing materials with others or using them commercially is prohibited.

8. Health and Safety Disclaimer

• No Emergency Service: The Program is not an emergency service. If you experience a medical emergency, call 911 immediately.
• Individual Results Vary: Weight loss and health outcomes depend on many factors, including your genetics, effort, diet, exercise, sleep, stress, and overall health. No outcome is guaranteed.
• Your Responsibility: You are responsible for listening to your body, working within your limits, and seeking medical care if needed.

9. Limitation of Liability

To the fullest extent permitted by law:

Weight Loss 3D™ and Coach assume no liability for:

• Indirect, incidental, consequential, or punitive damages
• Loss of profits, revenue, data, or reputation
• Any damages or losses arising from your use of (or inability to use) the Program or Services
• Results or lack of results from following the Program
• Errors, omissions, or inaccuracies in coaching materials
• Third-party service failures (payment processors, hosting, etc.)

Your sole remedy for dissatisfaction is to discontinue use of the Program. You understand and accept these limitations.

10. Modification of Terms

Weight Loss 3D™ reserves the right to modify these Terms at any time. Changes become effective when posted to our website. Continued use of the Program after changes means you accept the new terms.

11. Termination

Weight Loss 3D™ may terminate your account or access to the Program at any time, for any reason, including breach of these Terms. Upon termination, you lose access to all Program materials and services. Refunds are not provided.

12. Links and Third Parties

Our website may contain links to third-party websites or services (payment processors, social media, etc.). We do not control these services and are not responsible for their content, privacy practices, or performance. Use of third-party services is at your own risk and subject to their terms.

13. Disclaimer of Warranties

Weight Loss 3D™ and all Services are provided "as is" without warranties of any kind, express or implied. We do not warrant that Services will be uninterrupted, error-free, or secure.

15. Governing Law

These Terms are governed by the laws of the State of Ohio, without regard to conflict of law principles. Any legal action or proceeding must be brought exclusively in the state or federal courts in Franklin County, Ohio. You consent to personal jurisdiction in these courts.

16. Entire Agreement

These Terms and Conditions, along with our Privacy Policy and Program Disclaimer, constitute the entire agreement between you and Weight Loss 3D™ regarding the Program. If any provision is found unenforceable, the remaining provisions remain in effect.
